Filament is not reliably being reinserted into/through extruder

Now 35 hours into this install, Question 1: what advice do you have for a mk3 that will not reliably reinsert filament into/through the extruder.

notes:

- tips are shaped by 1.8mm tube and coupler and mk3 code

- I have a degree in Computer Science and learned GCODE for this and can confirm mine is good (probably)

- I purchased a BMG extruder clone which I have done your 2 fixes for (Q2: how tight should the hand screw be? mine is as loose as possible so the 3dchameleon can push fil past the spring force and still work)

- system running on ender 3 v2 with marlin firmware, with prusaslicer, prints perfectly by itself

It will enter and get caught by the extruder by itself the first few layers of a print, but then a couple tool changes later one of the filaments will eventually not actually insert all the way into the extruder/or the extruder gears won't catch it for some reason, which is print ruined. please help!

Here is all my pics/vids, including the video of the filament not inserting and a pic of where it was at in the extruder. sometimes it's before the gears, sometimes in the gears. currently I am pulling out all filament from the 3dchameleon and starting fresh with untouched filament in case that's the reason. Also here is my GCODE and printer config from prusa
